Pre-Registration/Medical History Submittal Required in Advance
To assist us in your safety and identification of any special needs or precautions for the surgeon, clinical team and anesthesia during your procedure we ask that you complete as far in advance of your surgery day as possible. For your convenience we offer One Medical Passport for your online submittal portal.
Online registration is fastest and most convenient way to submit your information, medical history and current medications list. If you do not have computer access you may also contact the Pre-Registration/Pre-Admissions Department at 321-729-9493 ext. 2543 or 2544.
